What is hyperhidrosis?

Hyperhidrosis is the presence of excessive sweating in the body irrespective of the hot weather. Some people may have excessive sweat soaking through their garments, even when the temperature is not hot, causing plight of embarrassment and disrupts normal daily activities. It may restrict social activities and may cause avoidance of physical contact. Hyperhidrosis usually begins in teens or may appear in adulthood.

What are the causes of hyperhidrosis?

The causes of hyperhidrosis can be primary or secondary.
The primary or essential cause of hyperhidrosis is mainly due to hyperstimulus of the nerves which are normally responsible for sweating, irrespective of any trigger by hot temperature. Any kind of stress or anxiety makes it worse.
Secondary hyperhidrosis occurs due to some underlying medical conditions. More common conditions are diabetes, menopausal hot flush, nervous system disorder, obesity, gout, thyroid conditions etc.

What to expect in preliminary consultation with our plastic surgeon?

Our plastic surgeon will take a detailed history and try to rule out any specific causes of hyperhidrosis. He will also ask for medical investigations to rule out any secondary causes for the same.
If the problem is being caused by an underlying medical condition then the focus will be to treat that condition first. Otherwise, focus is on treatment of hyperhidrosis as primary causes.

What are the treatment options for hyperhidrosis?

  1. Oral medications - Some oral medications, which are known as anti cholinergic drugs, block the chemicals that cause nerves to communicate with each other. This can reduce sweating in some people. Possible side effects include dry mouth, dry eyes leading to blurred vision and bladder problems.
  2. Botulinum toxin injections - These injections (Botox, Myobloc, others) temporarily block the nerves that are responsible for cause sweating. This is a very effective method of temporarily getting relief from excessive sweating. Each affected area of the body will need several injections. The effects last 6 to 12 months, and then the treatment needs to be repeated. Some people may experience temporary muscle weakness in the treated area.
  3. ETS (Endoscopic thoracic sympathectomy) – This surgical treatment is recommended only in cases that are severe and not responding to other above-mentioned treatments. The nerves that carry messages to the sweat glands are cut. ETS may be used to treat hyperhidrosis of the face, hands or armpits but avoided for feet because of the risk of permanent sexual dysfunction.
