How is scar formed?

Scar formation is our body's own healing repair process after any incision or trauma. If the underlying dermis is damaged, body makes more collagen to mend the wound and results in scar formation.
Natural healing and size of resultant scar depends on many factors such as person's genetics, race, skin quality, infection control, and wound location.

What are the types of scar?

There are various kinds of scar, important types are as follows:

1. IRREGULAR & PITTED SCARS - These types of scars do not impair function or cause physical discomfort and include acne scars as well as scars resulting from minor injury.
2. HYPERTROPHIC SCAR - They are often raised, red and may become wider over time. They result due to excess collagen production. they can be lighter than original skin or darker than original skin.
3. KELOID - these are little different than hypertrophic scar in that they grow beyond the original scar. they are more commonly present where there is little underlying fatty tissue, such as on the face, neck, ears, chest or shoulders.
4. CONTRACTURE - are scars that restrict movement due to skin and underlying tissue that pull together during healing. They are usually formed over a joint. This is the usual resultant scar which forms after a burn accident.

What is the correct time for scar revision surgery?

Scar revisions should not be done in haste. A scar should be allowed to mature first. Once the scar has healed, then the revision should be done. It may take from 3 months to upto 2 years for complete scar maturation (DEPENDING ON THE SCAR TYPE)

What are the treatment options available to lighten a scar?

Types of treatment options available are:

1. CHEMICAL PEELS - These are easy to apply facial like treatment being given on OPD basis. these chemicals act on superficial layer of skin which is epidermis at variable depth, cause its exfoliation and in turn, new layer grows from below in response to healing process.
   They work wonderful for lesions like acne scars.
2. DERMABRASION – This is mechanical polishing of the superficial layer of skin, exfoliates it and new smooth skin usually grows from underneath. It evens out surface irregularities such as deep lines or acne scars
3. LASER - These techniques resurface the skin to produce a smoother and more even skin tone. These methods are best for shallow scars and depressions. Nonablative lasers, such as the 585 nm pulsed dye laser, 1064 nm and 1320 nm Nd:YAG, or the 1540 nm Er:Glass are used as laser therapy for hypertrophic scars and keloids. Ablative lasers such as the carbon dioxide laser (CO2) or Er:YAG offer the best results for atrophic and acne scars.
4. CARBOXY THERAPY – This is a quick and painless coffee break treatment which involves administration of gaseous CO2 into the subcutaneous plane through a fine needle. This causes arterial vasodilatation, induces growth factors, helps in neocollagen and microvessel synthesis, lipolysis etc.
   It works wonder in episiotomy scars, post caesarean scars and breast surgery scars.
5. STEM CELL THERAPY- PRP or Platelet Rich Plasma is the autologous biotherapy which is used for scar remodelling and healing. It is prepared from one's own blood and is rich in growth factors required for regeneration of underlying tissue.
   It can be injected in new scarline during closure of new wound as well as healed scars.
6. STEROID INJECTION - A long-term course of corticosteroid injections into the scar may help flatten and soften the appearance of keloid or hypertrophic scars. Topical steroid applied immediately after fractionated CO2 laser treatment is another very effective option.
7. HYALURONIC ACID - These are dermal fillers which are injected at periodic intervals to enhance depressed scars or acne scars.
8. LED THERAPY – Light emitting diode causes photobiomodulation of cells resulting in improved inflammation, pain, scar healing. It works beautifully in episiotomy healing or scar remodelling.
9. OTHERS - Silicone Gel sheets –These can help flatten hypertrophic and keloid scars.
10. SURGICAL SCAR revision - This option is often combined with other above mentioned techniques to give you the best results. Re surgery done is through finer sutures and surgery time is kept minimum. Microscopic loupe is used for magnification and surgery is done in avascular planes with very fine instruments and achieving atraumatic dissection so that tissue damage is minimal.
